Stenograph

Medical dictation software for clinicians who write all day

Stenograph is medical voice recognition software for physicians, nurses, therapists and other clinicians who spend the end of every clinic day catching up on letters and notes. You speak the referral letter, the report or the progress note, and the text lands formatted in the app you document in, with medication names, anatomy and procedures spelled correctly from a medical vocabulary and your own term list.

Clinical documentation by voice instead of after hours

Clinicians often describe documentation as the part of the job that follows them home. A referral letter that takes ten minutes to type can be spoken in a fraction of that time, because most clinicians speak far faster than they type. The catch with general dictation has always been the vocabulary: a misspelled drug or a garbled eponym turns a quick dictation into slow proofreading. Stenograph is built around getting those words right and putting them into a usable shape.

  • Referral and discharge letters
  • Clinic letters to patients and other providers
  • Progress notes and assessment summaries
  • Procedure and imaging reports

Medical voice recognition software that knows clinical terms

The medical vocabulary covers drug names (generic and brand), dosing language, anatomy, procedures, eponymous signs and common abbreviations. On top of it you keep your own list: the surgeons you refer to, local service names, the products and devices you prescribe. Add a term once and every later dictation spells it your way. Pro, Team and Enterprise plans include every professional vocabulary, and Solo includes one of your choice. The page on custom vocabulary speech recognition shows how terms are managed.

  • Medication names, strengths and routes spelled as written in prescriptions
  • Anatomy, laterality and procedure names
  • Your colleagues, clinics and services in your own spelling
  • Shared term lists for a whole practice on Team and Enterprise

Clinical letter format without retyping

Dictate the content in the order you think of it and Stenograph shapes it as a letter: date and recipient line, salutation, paragraphs for history, findings and plan, and a sign off. Say "make that a list" when you read out a medication list and it becomes one item per line. Say "new paragraph" before the plan. If a sentence comes out wrong, "delete the last sentence" or "replace twice daily with three times daily" fixes it without touching the keyboard.

  • Letter structure with greeting, body and sign off
  • Medication and problem lists as real lists
  • Headings such as History, Examination, Impression and Plan when you speak them
  • Voice edits that act on the text you just dictated

Dictate into the system you already use

Stenograph inserts text at the cursor of whatever app is in front of you: a desktop EHR client on Windows, a browser based record through the Chrome extension, Word for longer letters, or the phone keyboard between appointments. It does not connect to your EHR through an integration; it types where you type, which means no IT project is needed to start. For longer pieces in Word, see dictation software for microsoft word.

How audio and text are handled

We describe our data handling exactly, because a clinician needs facts, not badges. Stenograph does not claim HIPAA certification and does not offer a Business Associate Agreement. Here is what happens instead.

  • Audio is processed and then deleted, it is not stored

  • Text history is optional, you can switch it off and delete it whenever you like

  • Your content is never used to train AI models

  • Data is encrypted in transit and at rest

  • The microphone is active only while you dictate, never in the background

Because of this, follow your organization's policy on patient identifiers. Many clinicians dictate the clinical content with Stenograph and add names, dates of birth and record numbers from the record system itself. If your organization requires a signed BAA for any tool that touches patient information, use a product that provides one. The dictation privacy page has the full detail.

Where medical dictation with Stenograph fits best

Stenograph suits clinicians in private practice, outpatient specialists, therapists, consultants writing reports and anyone whose documentation includes long letters. It is a writing tool: it does not listen to patient visits and does not write notes from an ambient conversation. If you record a consultation with consent and need it turned into a letter, the companion page on medical transcription software covers file based work.

  • Specialists dictating referral replies and clinic letters
  • Physiotherapists, psychologists and dietitians writing reports
  • Practice managers and clinical leads preparing policies
  • Medical writers and reviewers working on long documents

Questions clinicians ask about medical dictation

Is Stenograph HIPAA compliant

We do not claim HIPAA certification and do not sign a BAA. Audio is deleted after processing, text history is optional and deletable, and content is never used for training. Follow your organization's policy on patient identifiers.

Will it spell medication names correctly

The medical vocabulary covers drug names, anatomy and procedures, and your own term list adds anything specific to your practice. Every term you add is spelled your way in later dictations.

Does it integrate with my EHR

It does not use an EHR integration. It types at the cursor, so it works in a desktop EHR client, a browser based record or a word processor.

Does Stenograph record my patient consultations

No. The microphone is on only while you press the hotkey and dictate. There is no ambient listening.

Can I try it before paying

Yes, there is a one-time demo on the home page. Plans start at 24 USD per month billed yearly, and no plan is priced at zero.
